IF YOU ARE NOT A CITY EMPLOYEE...

Private citizens and third parties cannot, by law, file claims directly with OMAG. Instead, you must contact the Municipal Clerk of the Municipality involved. They will ask you to fill out and sign the Notice of Tort Claim form. After the Municipality has accepted and reviewed this form, the Municipality will file this form with OMAG.

TORT CLAIM / GENERAL Liability Claim

Please note: Liability Claims from third parties CANNOT, by law, be accepted directly by OMAG. Municipal Clerks must file the appropriate documentation.

OMAG has updated the standard Notice of Tort Claim form. The new form includes 2 pages which can be completed by a person wishing to file a Tort Claim as well as the 3rd page which should be completed by you or your staff whenever a claim is received.

Whenever a tort claim is received, the claim along with any additional document or photos you retain should be submitted to OMAG as soon as possible after the claim is received. Claims can be submitted to OMAG by mail, fax (405.657.1401), by email (claims@omag.org) or by submitting the form online via the following link:

Municipal Clerks should provide Claimants a copy of Section A of the Notice of Tort Claim form. This information must be filled out by the Claimant. Municipal Clerks must fill out Section B. After all sections are filled out and signed by the appropriate parties, municipal staff should mail, fax, or email to the OMAG Claims Department or claims@omag.org.

If you have an active claim with OMAG and need to provide us a photo from your cell phone, you can text it to us at 405-657-1400. Make sure to include your claim number provided in your acknowledgment letter.


Automobile Claim

If you have received a tort claim related to an auto accident, that claim can be submitted by clicking the link above for submission of tort claims. If you want to file a claim related to damage to your vehicle or equipment, you can contact our Claims department by phone (405.657.1400) or email (claims@omag.org) to obtain the necessary forms for submitting the claim, or you can complete the form online:

For Automobile Collision Claims, OMAG strongly recommends all parties utilize the Oklahoma Traffic Collision Report Form. You can obtain a copy of this form that can be completed on a computer or by hand, as well as the instructions for completing the form on the Oklahoma Department of Public Safety website. Property Claim

If you have a municipal building that has been damaged by theft, fire, windstorm, or other loss, you may contact our Claims department, or press the Property Loss Claim Form button to submit a notice online. Either of these steps will start your claims process.

Workers' Compensation Claim

Workers' Compensation claims are administered by Consolidated Benefits Resources. Their contact information can be found on the Workers' Compensation policy information page. To file a Workers Compensation claim, claimants should utilize the copy of the Workers' Compensation claim form provided on City's USB flash drives during policy renewal. A copy of the Workers' Compensation claim form packet should be submitted to the offices of Consolidated Benefits Resources.
