IF YOU ARE NOT A CITY EMPLOYEE...
Private citizens and third parties cannot, by law, file claims directly with OMAG. Instead, you must contact the Municipal Clerk of the Municipality involved. They will ask you to fill out and sign the Notice of Tort Claim form. After the Municipality has accepted and reviewed this form, the Municipality will file this form with OMAG.

General Liability/Tort Claim
Please note: Liability Claims from third parties cannot, by law, be accepted directly by OMAG. Municipal Clerks must file the appropriate documentation.
OMAG requires the submission of a standard Notice of Tort Claim form.
Municipal Clerks should provide Claimants a copy of Section A of the Notice of Tort Claim form. This information must be filled out by the Claimant. Municipal Clerks must fill out Section B. After all sections are filled out and signed by the appropriate parties, municipal staff should mail, fax, or email to the OMAG Claims Department or email@example.com.
