IMPORTANT NEWS FOR HEALTH PLAN PARTICIPANTS!

The Insure Oklahoma/O-EPIC program is now available to our member cities with fewer than 50 full-time employees.  This program assists Oklahoma small employers in helping qualified lower to moderate income employees and spouses with health premium subsidies.  Unfortunately, OMAG has just been informed that our Plan 2000 does not meet the qualifications for the Insure Oklahoma/O-EPIC program.  We are investigating options to rectify this situation.  Our Plan 300, Plan 750, and Plan 1500 DO meet the qualifications for the program. 

Please visit the website at www.insureoklahoma.org and you’ll find OMAG listed there as a qualified health plan!  Follow the directions to download an application.  You may also call toll-free at (888) 365-3742 to have the forms mailed to you.  OMAG’s identification number is 6624.  Where you’re asked to indicate who your “agent” is, you’ll need to list the Town Clerk, City Manager, or whoever handles your city’s health insurance program.  OMAG has provided herein the necessary information asked for in #6 concerning the final rate schedule for the Qualified Health Plan selected.  These rates do not reflect those on your MAA/OMAG invoice but do comply with Insure Oklahoma/O-EPIC’s format.
